– then PT returns to your office for records appointment, impressions, photos and facebowLABORATORY
- Models, CT scan to lab – treatment plan case
- Edentulous PT – models, CT scan to lab – radiographic guide
– PT comes back to LOS office for new CT scan with radiographic guide in place – then TX plan case - Lab fabricates surgical guides and upper and/or lower prosthesis
- Lab should fabricate a night guide for PT to wear while in healing phase of treatment
- Restorative – edentulous – try in new prosthesis – check bite, shade, teeth etc. for any changes to communicate to lab

Surgical Appointment – Dr. Lane, Restorative, Laboratory
- Dr. Lane’s office to have all instrumentation and abutments for case
- Extraction of remaining teeth, place implants, grafting as needed
- Place prosthesis and adjust occlusion
- Restorative Office should see PT for occlusal adjustment within 2 days of surgery and 4 weeks after surgery to check bite
- Dr. Lane should see PT 2 weeks after surgery for post op check

Final Prosthesis Impression / Implant Torque Test Appointment
- Dr. Lane, Restorative Doctor and Lab in Plymouth office
- Remove prosthesis by restorative
- Torque test all implants by Dr. Lane
- Place impression copings and take impression by Restorative Doc and laboratory
- Communicate any changes to laboratory regarding final prosthesis
- Lab to fabricate framework / wax up

Framework Try-In Appointment
- Restorative Doctor and Laboratory to try in framework in Plymouth Office
- Verify teeth shade, size, shape, bite
- Replace prosthesis

- Remove temp prosthesis
- Place fixed prosthesis
- Photos and CT scan
